1. Application form fully completed and signed by applicant 
2. One recent passport-size Photograph (white background) 
3. Passport/Travel Document must: 
   a) Have been issued within the previous 10 years, and its validity must not exceed 10 yrs. 
   b) At least have a validity of 3 months after intended stay. 
   c) Have at least 2 Blank Pages. 
4. Cover letter 
5. Overseas medical insurance with minimum coverage of Euro 30,000 - for emergency hospital and repatriation expenses. (Photocopy) 
6. Travel itinerary with travel dates, including a return flight reservation or ticket (Please mention if any other country/countries are to be covered during the entire Trip). 
7. Two set photocopies of Applicant’s Passport. 
8. Evidence of applicant’s employment: 
   a) If the applicant is an employee: Headed paper company’s original approved letter, latest 3 months pay slips and latest 3 years Income Tax Return. 
   b) If the applicant is Self-Employed: Full Business Certificate of Incorporation + last 3 months Business Bank Statement + latest 3 years Income Tax Return 
   c) If students applying: Original letter from the school, dated within 30 days before date of submission, must report: Full name of the applicant, full name and address of school, telephone number of the school, signature of the Head of Department. 

9. Availability of adequate financial means of the applicant 
   a) Salary slips/certificates of the last three months 
   b) ITR with Barcode 
   c) Personal bank statement for the last six months, in original letter stamped from the bank or then the online print of bank statement should be stamped and signed from the branch manager. 

ALTERNATIVELY 
Original and one photocopy of Bank Guarantee (“fidejussione bancaria”) from sponsor in Italy (if any) covering the expenses for the duration of stay as per the foreign exchange value (sse above mentioned table) accompanied by personal bank statements. The original bank Guarantee will be returned with the passport.

9. Proof of Accommodation: Confirmed hotel bookings.

10 Copy of all previous Schengen Visa

11 For Minors: 
   a) Application to be signed by both parents. 
   b) If applicant is traveling with both parents: birth certificate. 
   c) If applicant is travelling with one of the parents: original consent letter written and signed by the other parent, legalized through the Italian Embassy/Consulate
   d) If minor is travelling alone: original consent letter to travel abroad written and signed by both parents, legalized through the Italian Embassy/Consulate/Municipality where parents    are resident If one of the parents. Passed away, death certificate (original and copy) together with translation in English. If parents are resident in India they will have to sign the          consent letter at the Consulate.

•    My passport is about to expire. Can the visa still be issued?
No. Your passport must be valid for minimum of 6 months provided the passport is valid for at least 3 months from the date of departure from the Schengen Area.

•    My passport has run out of blank visa pages but I still have empty pages for endorsements etc. Can I apply for a visa?
No. You need at least 2 blank back to back visa-pages in your passport when applying for a visa. The visa sticker can’t be placed on pages not specifically reserved for visas.

•    Where can I submit my application?
Applications can be submitted at nearest VFS Global office with prior appointment

•    Am I required to submit my application in person at VFS Global office?
Yes, all applicants (including minors) are required to submit their applications in person.
